what to bring:

For your first visit, it is absolutely necessary that you bring your insurance card, driver license or government issued ID, a copy of your referral if required by your insurance, and a list of your symptoms.

waiting list:

There is typically a 1-2 month waiting list for new patients.

patient responsibility:

It is the patient's responsibility to research with their insurance provider on whether Dr. Namey is listed on their individual plan as a provider. Contact the number on the back of your insurance card. Patient is also responsible for a referral from your primary care physician, if required by insurance.

cost to you:

The co-pay amount for a specialist visit is due at the time of check in. Patient is also responsible for their deductable.
